The blindness is never total. Either the attendant or the patient will pick up clues as to which is the placebo. Sometimes through inflections of voice or body language. Sometimes just by appearance.

Even after the study what about the person compiling the data. Rarely are they blinded as to what is being tested. They make decisions about excluding outliers ,applying statistics, and what the results will be compared to.
